Every form in the packet is free from the agency that issues it: SS-5, DS-82, DS-5504, voter registration, Texas DL-14A, Texas 130-U, New York MV-44NC, New York MV-82, Pennsylvania DL-80, Pennsylvania MV-41A. The $49 pays for the pre-filling, the sequencing and the letters. Agency fees (passport $130 by mail, Texas licence $11, New York $12.50) are paid to the agency, not to us.

    Frequently asked questions

    Which forms are pre-filled?

    Form SS-5 (Social Security), form DS-5504 or DS-82 (passport, chosen by your passport's issue date), the National Mail Voter Registration Form, and the driver licence and vehicle title forms for Texas (DL-14A, 130-U), New York (MV-44NC, MV-82) and Pennsylvania (DL-80, MV-41A, voter application). Other states get step-by-step instructions and links instead of pre-filled licence forms.

    Does my Social Security number leave my computer?

    No. The forms are filled by your browser on your device. The only thing our server ever receives is the payment confirmation from Stripe. If you clear your browser data, your answers are gone; download the packet first.

    Is this a government service?

    No. PDFops is a private company and is not affiliated with the Social Security Administration, the Department of State or any DMV. Every form in the packet is free from the agency that issues it; the links are on this page. You are paying for the pre-filling, the sequencing and the letters.

    What if I am not happy with it?

    Reply to your receipt within 60 days and you get a full refund, no questions.

    What is not covered?

    Name changes by court order, after divorce, or through immigration; passports that must be renewed in person (form DS-11); names with characters outside the Latin alphabet, which the forms' font cannot print.
